2025-2026 Athletic Clearance

All physicals must be dated on or after May 1, 2025

Students who participate in the following activities must complete the clearance process before participating in physical activity:

ALL PHYSICAL FORMS MUST BE SIGNED BY AN MD, PA OR DO.  CALIFORNIA ED CODE DOES NOT ALLOW NURSE PRACTITIONERS (NPs) TO COMPLETE ATHLETIC PHYSICALS.

Please Complete all 3 Steps

1. Preparticipation Physical Evaluation (Both Pages)
   • Medical Eligibility Form
     • MUST be completed by a licensed MD, DO or Surgeon
     • MUST have physician signature AND office stamp
     • Kaiser physical forms are accepted
2. Go to athleticclearance.com
   • Complete ALL 5 sections
     • Medical Insurance coverage is REQUIRED
     • If needed, information is available in the Front Office or go to https://myers-stevens.com/enrollment-page/
   • For those students without an ID number, please enter  0000
   • Choose “Troy” from the list and “2025-2026” as the school year
   • Once completed, please print the “Confirmation Page” and sign
3. Scan both the Medical Eligibility Form (1 Page) AND the SIGNED Confirmation Page to the website.
